Why nothing on your website should go live without your approval

Confidence is not accuracy

The failure people imagine is a system that crashes and stops. That's the easy case. The real failure is quieter: the system is asked for something it can't quite do, reaches for the nearest thing it can do, does that instead, and reports success.

Nothing errored. Nothing looked broken. You find out three weeks later, from a customer.

That's exactly why a human has to be in the loop at the moment of publishing — not because people are better at making the change, but because people are much better at noticing it isn't what they asked for.

What “approval” actually has to mean

An approval step is only worth having if it's real. Three things have to be true, or it's theatre:

  1. The change must genuinely not be live yet. Not a published change with an undo button — an unpublished change with a publish button.
  2. You have to be able to see it. A diff of database rows is not a preview.
  3. Declining must be free. If saying no leaves your site half-changed, you'll stop saying no.

The part that surprises people

Requiring approval doesn't slow a product down. It forces the system to be honest about what it did — because a vague report survives when nobody checks, and doesn't when a human has to look before it counts.
